Nottinghamshire County Council closely monitored national and international developments as they emerged regarding the COVID-19 pandemic (including key dates, such as the World Health Organisation declaring COVID-19 a Public Health Emergency of International Concern (PHEIC) on 30 January 2020 and declaring the outbreak of COVID-19 as a pandemic on 11 March 2020). A comprehensive multi-agency local structure was established through the Nottingham and Nottinghamshire Local Resilience Forum (LRF) to respond to the COVID-19 pandemic, with Nottinghamshire County Council playing a key role in outbreak control, humanitarian assistance, logistics, health and social care, testing, transport and infrastructure, finance, economy, excess death, coordination of local authority services and recovery.
The Strategic Coordinating Group (SCG) is chaired by Nottinghamshire Police, sits at the apex of the COVID-19 response structure and is responsible for establishing and maintaining effective multi-agency arrangements to respond to the COVID-19 pandemic. The first meeting of the SCG took place on 13 February 2020. The first meeting of the Tactical Coordinating Group (TCG), which reports in to the SCG and is responsible for enacting the strategic objectives of the SCG and overseeing multiple response sub-groups and cells, took place on 12 March 2020, the day after the WHO declaration of the pandemic. Nottinghamshire County Council published its Local Outbreak Management Plan in June 2020. This has subsequently been refreshed in March 2021 and is available online.
The person or persons with overall direct control of co-ordinating the response.
The Director of Public Health has overall responsibility for the local outbreak response. Please see the COVID-19 contain framework: a guide for local decision-makers for further detail regarding local roles and responsibilities in relation to the prevention, management, and containment of COVID-19 outbreaks.

At the time of writing, there are 3 static asymptomatic community testing sites in Nottinghamshire County and 4 mobile testing units travel between a number of mobile testing sites, of which 11 are in Nottinghamshire County. All operational costs for asymptomatic testing sites are currently recharged to the Department of Health and Social Care. Costs are contained in overall COVID-19 spend for 2020/21 (see response above).
Symptomatic testing sites are coordinated and directly funded by the Department of Health and Social Care (DHSC).
Please state the number of COVID-19 vaccination centres/stations established and the cost of setting up and maintaining each. State how these costs are met.
Nottingham & Nottinghamshire Clinical Commissioning Group and Bassetlaw Clinical Commissioning Group are responsible for the delivery of the vaccination programme, working in partnership with their respective Integrated Care Systems.

All deaths where a patient has been compromised by the effects of Covid 19 are required to have Covid 19 somewhere on the death certificate.
This includes all deaths where patients have suffered the effects of Covid 19, where a positive swab has been returned and their Covid symptoms have directly contributed to the death. Covid 19 should then be written in Part 1 of the Medical Death Certificate. If a patient has had a positive swab in the recent past but has died of other causes then it is appropriate for Covid 19 to be placed in part 2 of the certificate, as a contributory factor. However, some patients are never tested for Covid 19 due to emergency situations etc or they have had a negative swab. These patients are then diagnosed with Covid 19 on a differential diagnosis. They are, diagnosed by particular changes in the lung tissue, appearances of which are specifically characteristic of Covid 19. The diagnosis of Covid 19 is given when these changes are seen alongside changes in blood results, known as Lymphocytopenia. Other characteristics include, Type 1 respiratory failure. The whole picture leads to the differential diagnosis. Therefore these collective characteristics are specific to Covid 19 and should be written on the Medical Death Certificate in these cases. |

Accountability – All Covid 19 deaths are reported to the CPNS (Covid 19 Patient Notification System) which is the National system for recording Covid 19 deaths. There is no requirement for these deaths to be reported to the Coroner unless other clinical or environmental issues dictate. Cost – Provision of body bags and PPE. |

Please find below/attached the Isle of Wight NHS Trust’s response to your request.
How many patients have died in the Isle of Wight with covid-19 as the only factor (eg) without comorbidity for the period 1st January2021 to 1st June 2021.
Response - The IWNHS Trust has reported all deaths as per guidance issued by NHS England and has Medically Certified Cause of Death (MCCD) in line with the Coronavirus Act March 2020 – excess death provisions: information and guidance for medical practitioners which sets out the clauses relating to death certification of COVID 19(C19) related deaths.
As of 1st June 2021, there have been 174 C19 reportable deaths the first one occurring in March 2020. These were registered on the NHS England C19 Patient Notification System (CPNS) as mandated in that there was a positive C19 swab result within 28 days of death and /or C19 was on the MCCD. Doctors must medically certify a death as follows:
Part I
(a) Disease or condition directly leading to death
(b) Other disease or condition, if any, leading to 1(a)
(c) Other disease or condition, if any, leading to (1b)
Part 2 Other significant conditions contributing to the death but not related to the disease or condition causing it
Of those 174 reportable deaths:
- 157 had a positive C19 test result within 28 days of death.
- 17 deaths had negative C19 test results but had high clinical suspicion of C19 therefore entered onto the MCCD as per clinical symptoms and the guidance.
- 165 deaths had COVID detailed on the MCCD (further detail below).
- 6 deaths are with HM Coroner so no MCCD issued to date
Of the 165 MCCD’s the Trust has issued/ been notified of to date:
- 135 had C 19 detailed in Part 1 (a) i.e the disease or condition directly leading to death
- 17 had C19 detailed in Part 1 (b) i.e other disease or condition, if any, leading to 1(a)
- 2 had C19 detailed in Part 1(c) i.e other disease or condition, if any, leading to (1b)
- 11 had C19 detailed in Part 2 i. e. Other significant conditions contributing to the death but not related to the disease or condition causing it

Please could you give me the number to deaths whereby Covid is the sole cause of death listed , there were no other pre-existing conditions or comorbidities noted on the death certificate, covering the period from March 2020 until present day. If there answer is any, are you able to provide me with the average age of the deceased?

Note: The above deaths are patients who solely had Covid-19 on their medical certificate of cause of death.
I would also like the number of cycles you run your PCR tests on normally as well as the number of cycles that you are running for PCR tests where the patient/recipient is known to have been vaccinated with one of the available Covid jabs.
Assays used within the laboratory run between 40 and 45 cycles.
If there is any difference in the number of cycles being used in relation to the patient's vaccination status please could you explain why?
There is no difference in relation to patient’s vaccination status.

I can now respond as follows:
How many people have died in Aberdeen Royal infirmary from covid 19 alone? That is they were admitted to hospital and died due to covid 19 and not any other reason. There were <5 individuals whose sole cause death was Covid-19 at Aberdeen Royal Infirmary in that time period. Due to the very small number involved the exact figure cannot be disclosed as this may identify individual patients; s.38(1)(d) of the Act refers – Personal information - deceased person’s health record.
Can you also tell me of those that have died of covid 19 how many tested positive for covid 19?
There were146 deaths at Aberdeen Royal infirmary and 302 across Grampian up to 31 December 2020. These include any deaths within 28 days of positive test or where Covid19 was mentioned on death certificate following a positive test. It does not include suspected cases.
Under section 20 (1) of the Act, if you are dissatisfied with the way NHS Grampian has dealt with your request, you have a right to request a review of our actions and decisions in relation to your request, and you have a right to appeal to the Scottish Information Commissioner.

Q1. Please provide data for the total number of people that have died in NHS Tayside hospitals from purely COVID-19, that is the number of people who have died of COVID-19 without any other known conditions, from March 1st 2020 to 31st December 2020, please exclude those that have tested positive for COVID-19 and died within 28 days but not confirmed that the deaths were due to COVID-19.
Q2. Please provide data for the total number of people that have died in NHS Tayside hospitals from purely COVID-19, that is the number of people who have died of COVID-19 without any other known conditions, from January 1st 2021 to April 30th 2021, please exclude those that have tested positive for COVID-19 and died within 28 days but not confirmed that the deaths were due to COVID-19.
Q3. Please state how many people have died in NHS Tayside hospitals within 14 days of receiving a vaccine, of both the 1st and the 2nd vaccine, for COVID-19. Please state the type of vaccine taken.

Q1. For those deaths occurring in NHS Tayside hospitals between the 1 March 2020 and 31 December 2020, 6 deaths were recorded where the cause of death was solely due to COVID-19. This is based on the information recorded on the death certificate and by the National Register Office for Scotland and may not reflect the medical history of each individual.
Q2. For those deaths occurring in NHS Tayside hospitals between the 1 January 2021 and 20 April 2021, 11 deaths were recorded where the cause of death was solely due to COVID-19. This is based on the information recorded on the death certificate and by the National Register Office for Scotland and may not reflect the medical history of each individual.
Q3. NHS Tayside are unable to provide this information as it is not held within NHS Tayside. To answer this it would involve linking patient level vaccination data (held centrally by Public Health Scotland) with individual patient deaths records.
As such, in terms of Section 17 of the Freedom of Information (Scotland) Act 2002, this represents a notice that the information requested for question three is not held by NHS Tayside. Please redirect this question to Public Health Scotland [email protected] who may hold this information.
